Administrative Assistant

REPORTS TO: CEO, Executive Director and/or Office Manager

Summary:
Administrative Assistant duties and responsibilities include providing administrative support to ensure the efficient operation of the office. Supports all staff through a variety of tasks related to organization and communication. Responsible for confidential and time sensitive material. Familiar with a variety of the field’s concepts, practices and procedures. Ability to effectively communicate via phone and email ensuring that all Administrative Assistant duties are completed accurately and delivered with high quality and in a timely manner.   • Answering the phone, take messages and greet clientele and external partners

Clinical Director

General Description:
Clinical Director is responsible for the oversight and development of clinical departments through tasks such as hiring, evaluating, scheduling, and supervision. The Clinical Director will provide strategies that allow for continued growth and provides clinical training to staff and interns. Licensed Clinical Supervisor

General Description:
Licensed Clinical Supervisor will provide clinical and administrative supervision to all Direct Care Providers; who are providing consistent, strength-based, evidence based, and culturally competent services to all clients enrolled in treatment. All Supervisor must be available to provide clinical support on an as needed bases. They review, assess and evaluate the appropriate level of care, of all clinical documents including; but not limited to treatment plans, safety plans, comprehensive assessments and CANS.

The Licensed Clinical Supervisor will provide individual weekly structured supervision to Direct Care Providers. They will teach train, evaluate, provide support, and discipline Direct Care Providers. This is to ensure that referrals meet medical necessity, and are appropriately opened and discharged; assessing medical charts and evaluating worker performance, ensuring worker productivity.

Community Support Program Worker (CSP)

REPORTS TO: Licensed Clinical Supervisor

Provides an array of services delivered by community based, mobile, paraprofessional staff, supported by a clinical supervisor, to members with psychiatric or substance use disorder diagnoses, and/or to Members for whom their psychiatric or substance use disorder diagnoses interfere with their ability to access essential medical services.

Summary:
Provides support services that are necessary to ensure Members access and utilize behavioral health services. CSPs do not provide clinical treatment services, but rather provide outreach and support services to enable Members to utilize clinical treatment services and other supports. The CSP service plan assists the Member with attaining his/her goals in his/her clinical treatment plan in outpatient services and/or other levels of care, and works to mitigate barriers to doing so.

 

Responsibilities:

  • Assist Members in improving their daily living skills so they are able to perform them independently or access services to support them in doing so
  • Provide service coordination and linkages
  • Provide temporary assistance with transportation to essential medical and behavioral health appointments while transitioning to community-based transportation resources (e.g., public transportation resources, PT-1 forms, etc.)
  • Assist with obtaining benefits, housing, and health care; Collaborating with Emergency Services Programs/Mobile Crisis Intervention (ESP/MCIs) and/or outpatient providers; including working with ESP/MCIs to develop, revise and/or utilize Member crisis prevention plans and/or safety plans as part of the Crisis Planning Tools for youth
  • Foster empowerment, recovery, and wellness, including linkages to recovery-oriented, peer support and/or self-help supports and services.
  • Document all sessions, encounters, and contacts made with clients and collaterals
  • Work with racially and socio-economically diverse populations

Master’s Level Clinician (IHT)

Provides an array of services delivered by community based, mobile, paraprofessional staff, supported by a clinical supervisor, to members with psychiatric or substance use disorder diagnoses, and/or to Members for whom their psychiatric or substance use disorder diagnoses interfere with their ability to access essential medical services.

The Clinician provides counseling therapy, crisis intervention, and case management services in a setting where the youth where the youth is naturally located, including, but not limited to the home (including foster homes), schools, child care centers, and other community settings, and other community setting. The Clinician cares for the needs of their specific patient population and provides appropriate direct patient care through assessment, diagnosis and treatment of specific diagnosed conditions that are billable.

 

Responsibilities:

  • Provides Intensive Family Therapy that may include working with the entire family (or subset of the family) to implement focused structural, strategic behavioral techniques or evidence-based interventions to enhance problem-solving skills, risk management/safety planning, communication, skill-building to strengthen the family, and to advance therapeutic goals to improve ineffective patterns of interaction.
  • Prepares CANS (when working with youth under 21), Assessments, Treatment Plans, Quarterly Reviews, Discharge Summaries, Progress Notes, follow-up referrals/care plans, other clinical documentation; provides therapeutic crisis intervention and urgent services as required.
  • Collects data about patients through interviews, case history, assessments, and/or observational techniques; evaluates data to identify and foster understanding of the family dynamic, utilizing strength based, culturally sensitive, age/developmentally appropriate teaching strategies to address stressors as they arise in order to determine the proper therapeutic approach and/or referral to other specialists, if required
  • Identifies and contacts community resources needed to support and sustain achievement of the youth’s treatment plan goals and objectives
  • Coordinates with and consults with collateral providers, state agencies, ESP/Mobile Crisis Intervention, and other individuals that may impact the youth’s treatment plan goals and objectives.
  • When requested by the family, will accompany the family to meetings regarding the youth’s behavioral health treatment needs in school, day care, foster homes and other community based locations
  • Obtain and document all required signatures and/or verbal consents deemed necessary in order to maintain compliance status
  • Assigns appropriate referrals for the Therapeutic Training and Support (TT&S), team member to follow through with appropriate social service agencies, financial assistance and other required services
  • Provide social and behavioral goals for the Therapeutic Mentor (TM) assigned to the case
  • Update intake/quarterly/bi-annual documents as prescribed to determine reliability of treatment
  • Establishes and maintains case files via electronic medical records, referrals, and other related documents for the treatment of clients; prepares related documentation for civil commitment hearings or other legal proceedings; keeps abreast of patient data to ensure appropriate treatment and care are delivered.

Outpatient Clinician provides behavioral health services that are rendered in an office, clinic environment, members home, or other locations appropriate for psychotherapy and counseling. The outpatient clinician provides counseling that focuses on the restoration, enhancement, and/or maintenance of a client’s optimal level of functioning, and the alleviation of significant and debilitating symptoms impacting at least one area of the client’s life domains (e.g., family, social, occupational, and educational). The outpatient therapist is responsible for developing a treatment plan with measurable outcomes, and a discharge plan that includes the identification of realistic discharge criteria that are developed as part of the initial assessment and treatment planning process and are evaluated and revised as needed.

Recovery Specialist

General Description:

Summary:
Recovery Specialist provides support for people with substance use problems. They teach individuals how to modify their behavior with the intention of full recovery. Because clients are susceptible to relapses, many Recovery Specialist work with clients on an on-going basis, individual, group and as needed.

 

Responsibilities:

  • Meet with clients to evaluate their health and substance problem
  • Identify issues and create goals and treatment plans
  • Clarify goals strategies, education and resources and access to treatment and recovery supports
  • Identify services and resources available in the community
  • Teach clients coping mechanisms
  • Help clients find jobs or reestablish their career
  • Lead group therapy sessions
  • Provide updates and progress reports to courts
  • Refer clients to support groups
  • Set up aftercare plans, that will follow the client for 6-12 months after discharge
  • Aftercare home visits for 6-12 months after discharge
  • Meet with family members and provide guidance and support
  • Document all sessions, encounters, and contacts made with clients and collaterals
  • Work with racially and socio-economically diverse populations
  • Basic computer skills that includes Microsoft Office, and the ability to create electronic medical records
  • Actively participate in staff meetings, in-service training, and all other agency sponsored events
  • Ability to work both independently and in a team

Qualifications/Requirements:

  • Two to Five years working with individuals challenged with mental illness/substance use. LADC11 or LADC Assistant.
  • Possess sufficient knowledge and understanding of treatment and recovery from substance use disorder
  • Ensure safety and structure of program milieu by monitoring clients activities, schedules, and assisting with personal needs
  • Ability to create a stable program setting and structure
  • Participate in care team meetings
  • Provides mutually supported relationships, strength-based approach, sharing experience and inspiration about recovery
  • Provide empathetic listening, ethics and practice supporting voice and choice   Basic computer skills that includes Microsoft Office, and the ability to create clinical documents
  • Ability to work both independently and in a team.

Therapeutic Mentor

Summary:
The Therapeutic Mentoring (TM) facilitates, teaches and reinforces developmentally appropriate social and behavioral skills, while providing a positive role model to youth under the age of 21. The Therapeutic Mentor provides structured, one to one strength based support services for the purpose of addressing the youth’s daily living, social, and communication needs Mentor The Therapeutic Mentor provides support and assistance, and/or engages the child in enriching community-based experiences. The Therapeutic Mentor works one-on-one with the youth, enlisting information and feedback from his/her Parent/Caregiver, Individual/In Home Therapy Therapists, in order to implement treatment goals.

  • Engage the youth in activities in the community or home that are designed to address one or more goals on a youth’s outpatient or In Home Therapy Treatment Plan;

  • Develop and maintain CBHI/CMHWC policies and procedures relating to all components of Therapeutic Mentoring Services;

Qualifications/Requirements:

  • Must be 21 years of age or older, and have the following education and experience, Bachelor’s degree in human’s service field and one-year experience working with children/adolescents/ transition-age youth; or an Associate’s degree in a human service field and 1-year experience working with the target population; or a high school diploma or GED and a minimal of 2 years’ experience working with the target population
  • TM must have a valid Massachusetts Driver’s license, reliable vehicle, and ability to work in community and in-home settings.
  • TM is required to meet with the Hub at least once a week to provide updates, feedback and ongoing information about the client;
  • TM must attend supervision meeting with assigned supervisor for 1 hour, per week, to discuss case;
  • Basic computer skills that includes Microsoft Office, and the ability to create electronic medical records

Summary:
Therapeutic Training and Support Provider (TT&S) works as part of an In-Home Therapy (IHT) team that includes an In-Home Therapist, for the purpose of treating a youth’s mental health, behavioral and emotional needs, including improvement of the family’s ability to provide effective support for the youth to promote his/her healthy functioning within the family.

  • Work under the supervision of an In-Home Therapist to support the implementation of a treatment plan designed to assist the youth and the family in achieving the goals and objectives of the plan.

  • Teach the youth to understand, direct, interpret manage, and control feelings and emotional responses to situations, and will assist the family to address the youth’s emotional and mental health needs.

  • Basic computer skills that includes Microsoft Office, and the ability to create clinical documents
  • The TT&S is required to meet with the team at least once a week to provide updates, feedback and ongoing information about the client;
  • Required to meet with the family for weekly counseling sessions;
  • Document all sessions within 24 hours of all encounters, and contacts made with clients and collaterals;
  • Ability to work both independently and in a team.
